Health Club Membership Reimbursement Policy
Village employees and spouses are eligible for reimbursement up to $25 per month household maximum for all health and fitness reimbursements or Village recreational exercise classes. The employee and spouse must participate in the Health Risk Assessment (HRA) and meet with the onsite nurse to discuss the HRA results to be eligible for reimbursement.
How to get Reimbursed
Submit the following information along with the health club reimbursement for to the Village Finance Department:
- Name of the health club
- Type of membership
- Monthly fee (reimbursement is half of the monthly fee, up to $25)
- Health club attendance record (from health club) showing at least 8 visits per month
- Health club payment receipt (showing proof of payment to health club)
- For Village recreational exercise classes, submit proof of payment and verification of attendance of the class
Reimbursements will be part of a regular paycheck and taxes will be deducted per IRS regulations.
